My vote

1) first choice: 3; second choice: 1
2a) yes
2b) yes (I don't think the content will be different depending on the
error, an error code should be enough)
2c) no

Reason: I think that having the "power" to create messages with its own
content we shouldn't loose the flexibility to do so. However, I
understand that option 1 may speed up adoption by being closer to
existing implementation so that's my second approach.
